Essentia Health Duluth MN is Minnesota's largest rural healthcare provider and offers services in MN, WI and ND. An integrated ACO, telehealth capability has been established in over 70 clinics, 17 hospitals and 6 nursing homes over the last nine years when the telehealth program was started. Psychiatric and behavioral health care has been one of the programs most active services but also experiences the most challenges due to the scarcity of providers. Elderly residents in nursing homes with dementia as a disability have often gone with a less than optimal level care across the US due to transport difficulties, disruptive behaviors and lack of specialist expertise. This phenomenon has resulted in many elders having difficulty finding a facility that is ready and accepting of their needs. This presentation will introduce two telehealth business models being used in MN by Essentia Health that is impacting this critical area of need . Program details and the steps needed to set up a replicated service will be explained by one of MN's pioneers in telehealth and rural patient care innovation. Participants will have an opportunity to ask questions about costs, facility staffing needs, equipment, reimbursement and program set up timelines.
